Adult Health Maintenance

 

This service is available in the home.  Nurses can provide monitoring of health conditions and assessment of patient whose condition is stable.  Aide services may also be provided if necessary.  Education on health conditions and assessments for additional services are also provided.

 

HOW TO APPLY:  Anyone can apply.  Our nurse will obtain MD orders for health monitoring.

COST:  No set fee; donations are accepted.  Aide service can be covered by the Senior Center Block Grant or Medicaid Waiver Program.
