HIV Case Management

 
     Case management, which is by definition, comprehensive and long term, is a mechanism for serving people with chronic conditions who have multiple service needs or who lack systematic access to needed services.  Case managers provide limited services (venipuncture, assessment, and education), help clients find services to meet their individual needs, help them coordinate their own care, help find ways to fund needed services (Ryan White, Title II grant, ADAP-AIDS drug assistance program, SSI, SSDI, Medicaid, Medicare), advocate for clients with bill collectors as well as in the medical system, and try and assure the best available care is being provided to the client.
