LT 101

 

     The LT-101 is a functional assessment to determine appropriateness of nursing home admission or community based waiver services in the home.  A person must get a minimum of 13 points on this assessment.  If they do not, the PHN works with the client, family or social worker to determine alternative plans for placement.

HOW TO APPLY:  Call our office to make a referral.  These usually come from hospital social workers, nursing homes, Dept. of Family Services or the family.

COST:  No cost to the client for assessment

 

CLINIC TIMES AVAILABLE:  Assessment is scheduled by the nurse
